Wheelchair Loan Form

WHEELCHAIR BORROWER AGREEMENTI understand that I am entirely responsible for the return of the wheelchair that I am borrowing from Halifax Shopping Centre (HSC) and I agree, by signing below, to return the wheelchair on loan to HSC Guest Services. I also recognize that I am financially responsible for any damage whatsoever caused to the wheelchair while in my possession.I agree that I will return the wheelchair to HSC Guest Services no later than 15 minutes prior to the mall closing, on the date borrowed. I agree that failure to return the wheelchair to HSC Guest Services by the scheduled time may result in the replacement fee for any item(s) not returned.I acknowledge that the wheelchair is not to be removed from Halifax Shopping Centre property. I acknowledge that wheelchairs are at no time, allowed on escalators or outside of the premises - they must remain inside of the shopping centre. I further agree to release, indemnify, and hold harmless OPB Realty Inc. - Halifax Shopping Centre, together with their officers, employees, agents, mortgagees, successors, assigns and all other whom it or they are in law responsible collectively, from any damages or claims arising in connection with my use of the wheelchair, including any claims arising from damage, loss or theft, or possible injury regardless of how it arises or is caused.Please return the wheelchair clean and in the same condition it was lent. Rentals are restricted to individuals 18 years of age and older.Thank you.
